Registered Nurse Resident Care Coordinator
Location: Based in the local county jurisdiction served by our government entity

Job Summary
As a Registered Nurse Resident Care Coordinator, you will play a vital role in coordinating and overseeing the care and well-being of residents within our county's healthcare system. Leveraging your specialized nursing expertise, you will work closely with interdisciplinary teams to ensure seamless, patient-centered care for our community's most vulnerable populations.
Key Responsibilities
- Assess and monitor resident health status, developing and implementing individualized care plans
- Collaborate with physicians, therapists, and other healthcare providers to coordinate comprehensive care
- Oversee the work of nursing assistants and other care team members to maintain high standards of resident care
- Document resident progress, communicate with families, and participate in care plan meetings
- Provide direct nursing care and support to residents as needed
- Ensure compliance with all applicable healthcare regulations and policies
Qualifications
- Active registered nurse (RN) license
- Minimum 2 years of experience in long-term care, geriatrics, or a similar nursing specialty
- Strong clinical assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-centered approach to care
- Familiarity with electronic medical record systems
Working Conditions
This position is based in a nursing home or long-term care facility within our county. The work environment can be physically demanding, with frequent standing, walking, and assisting residents. Flexibility to work a variety of shifts, including weekends and holidays, is required.
